1. Please fill all parts of the form, then click 'file', 'save as' and rename the file using the name of your school and followed by your name e.g. "XU-Ethelbert Deguit". Please Save as Microsoft Word Format and email to. Each school should submit the forms of its delegates together at the same time to avoid confusion. Submission should be made on or before 31 March 2010.

2. The Registration fee of (4,500) per participant and membership fee of (1,000) per school should be paid on or before 31 March, 2010. You should pay the fifty percent (50%) of the total registration fee on or before 28 January 2010. If you choose to pay the full participation fee on or before 28 January 2010 you may avail of a ten percent (10%) discount. Kindly inform Mr. Roberto B.
